Spasmodic dysphonia 

(also known as laryngeal dystonia) 

Spasmodic dysphonia is a neurological disorder that falls under the category of dystonias. During speaking, there are spasmodic contractions of the muscles involved in voice production. The severity of the condition can range from mild forms, characterized by voice breaks, to severe forms that make verbal communication impossible. The consequences include social isolation and occupational disability.

There are different types of spasmodic dysphonia:

Treatment:

The temporary suppression of muscle overactivity through botulinum toxin (Botox) injections has been considered the treatment of choice for this condition for decades. With experience, the injections can be performed painlessly under local anesthesia. The injection can be done externally through the skin under electromyographic (EMG) guidance, through the nose under flexible endoscopic control, or indirectly through the mouth under visualization. Rarely, the injection is performed under general anesthesia. Most patients can go home directly after a 15-30 minute treatment duration, and hospitalization is extremely rare.

Our experience shows that injections at the level of the vocal fold pockets have a comparable effect and lead to less voice weakness as a side effect.

Alternatively to Botox injections, there are surgical procedures available for the adductor type that promise more long-lasting results but come with associated risks and can only be performed under general anesthesia.
